Human Trafficking Consumer Dispute Resolution
If you are a victim of human trafficking seeking to block or dispute information related to your human trafficking experience, please contact Impact Employee Solutions’ Compliance Team at (866) 896-0603 or (602)761-0882.
To assist us in processing your request, please provide a copy of your Victim Determination Document, a document that determines you are a victim of trafficking, made by one of the following:
- a Federal, State, or Tribal governmental entity,
- a court of competent jurisdiction, which includes certain documents filed in court,
- a non-governmental organization or human trafficking task force, including victim service providers affiliated with these entities, authorized by a Federal, State, or Tribal government entity, or
- a self-attestation by a consumer who identifies as a victim of trafficking that is signed or certified by a Federal, State, or Tribal government entity, court of competent jurisdiction, or an authorized representative of these entities.
Please also include a List of Adverse Items of Information, documentation that identifies items of adverse information that should not be furnished by a consumer reporting agency because the items resulted from a severe form of trafficking in persons or sex trafficking of which you are the victim. Please include details on what specific information you are seeking to block or dispute.
In addition, please indicate your Preferred Method of Contact, either the mailing address or email address that Impact Employee Solutions should use to communicate with you in writing about your request.

Information Blocking Process
Once submitted, Impact Employee Solutions will initiate a block of the relevant information within four (4) days of receipt of your request. Our team will notify you within five (5) days if any additional information is required.
Impact Employee Solutions has twenty-five (25) days to make a final determination on your request. You will be notified within five (5) days of our determination to either maintain the block on your information or to rescind the block.
